Update your payment method
We don’t want you to lose the valuable coverage you have through the State Sponsored Life Insurance program, which currently is paid out of your National Guard paycheck.
To change your payment method to either checking account withdrawal or a bill sent directly to your home, please print, complete, sign, and return this form at the address listed at the top of the form. Remember you can keep this coverage until age 70, and there is no premium increase when you leave the National Guard.
